1. What you are acknowledging
This Client Acknowledgment is between you and Petri Wai. It sits on top of the Terms of Use and the Privacy Notice. You accept it by checking the box on the booking form. The email that then opens is a record of that acceptance.
It is not informed consent for psychotherapy. Only the provider can obtain that, and only after the provider has agreed to see you. Do not rely on this page as a consent to treatment.
2. Who provides care
Petri Wai (“Platform Operator”) provides the website, software, video-room links, and payment tools of Innerbloom Mind and Wellness at innerbloommindandwellness.com. The Platform Operator is an information-technology service provider only. The Platform Operator is not Rejean M. Lopez, is not any other mental-health professional, and does not practice psychology, counseling, medicine, or any other health profession.
The professional services, if any, are provided by the independent provider named in your request. As of 8 October 2026, the provider offered on this site is Rejean M. Lopez, RPsy, RPm, LPT, CHRA, practicing on her own behalf and not as an employee or agent of Petri Wai. Other licensed providers may be added later. Each one is responsible only for that provider’s own clients.
You do not have a therapist-client relationship with Petri Wai. You do not have one with a provider until that provider clearly accepts you.
3. Telehealth has limits
Online sessions can drop, lag, or be overheard by people near either device. They are not the same as sitting in a private office. They may be a poor fit in a crisis, when someone may be at risk of harm, or when a hands-on or local service is needed.
You are responsible for choosing a private place and a device you trust. The provider is responsible for deciding whether telehealth is clinically appropriate and whether that provider may serve you in the place where you are physically located during the session. Petri Wai does not make those decisions and does not verify your location.
A Philippine license does not automatically authorize practice everywhere a client happens to be. If you are outside the Philippines, ask the provider, before a session, whether they can work with you there.
4. Emergencies
You will not use Innerbloom Mind and Wellness for an emergency. If you are in danger, you will contact local emergency services or a crisis line, not the booking form and not a video room.
You understand that no one at Petri Wai is on call, that sessions are not monitored, and that a video room can sit empty.
5. Fees
The session amount shown on the site is a professional fee quoted for the provider’s service, unless the provider tells you otherwise in writing. Paying it, or requesting a time, does not by itself start treatment.
Petri Wai does not keep that professional fee as its own money. Questions about receipts for care, insurance, superbills, sliding scale, and refunds of the professional fee go to the provider.
6. Confidentiality is the provider’s duty
Confidentiality of counseling is the provider’s professional and legal duty, not a duty of Petri Wai. A provider can explain the limits, which often include a serious risk of harm, abuse or neglect of a child or vulnerable person where reporting is required, a court order, and supervision or consultation rules of that profession.
Petri Wai does not promise that email or a third-party video room has the same confidentiality as a provider’s locked record. Do not write sensitive detail you would not want sitting in an ordinary inbox.
7. No recording
You will not record a session, or allow another person to listen or watch, unless the provider and every participant agree in advance. Secret recording can be unlawful.
8. If you do not agree
If you do not agree, do not check the box and do not send a request. You may still use public crisis numbers. Nothing in this acknowledgment stops you from seeking care somewhere else, in person or with another professional.
